I have searched and search but can not find a definitive answer. I am trying to see if I should in camera NR. If ACR does not recognize and follow the file instructions , it would be a waste of time.
If NR in camera actually makes the adjustment to the NEF file, then maybe I would use it camera.
Or....Is it better to only use NR in ACR?

All camera settings are ignored by ACR. Apply your own noise reduction.
There's no way camera settings can be transferred. The processing algorithms are totally different, so it wouldn't make any sense.
The only exception is the white balance, but even that is just reverse-engineered to produce a visually similar result. The numbers won't necessarily be the same.

Not if it's done in the camera firmware - which is just a raw converter on a chip. You're replacing that with ACR.
A raw file is just a record of the photons that hit the sensor. That's all. The rest is processing, stored in metadata, either done in the camera or by a standalone raw converter.
Same argument applies for any camera setting. The algorithms are different. You can't simply take a parameter from one and apply it to the other - even if it made any sense to do so, which it doesn't, it wouldn't produce the same result.
This is why people are always surprised when their white balance figures don't carry exactly over. If they did, it wouldn't look right. To look right, the numbers can't be the same. That's a choice made by the ACR engineers, and the only one they could make. And that's the only camera setting they made a decision to incorporate - probably because people would protest too much if they didn't.
