choppy image

hello I have a problem with my premier pro.  it lags, and it skips frames. I get choppy images on both sides of the screen. how can I fix this?

2. What is the exact dot version of Premiere Pro? FAQ: How to find the exact version of Premiere Pro you're using?

3. What is the footage file format & codec? What is the source of the footage(Camera model, Software)?
