If you are opening the web page in Acrobat, select the options (or similar name like Conversion Settings in AA5) button at the bottom of the opening window. You should also be able to set these in the EDIT>Preferences under the "to PDF" tab. -

Let me tell you how I do it.
I found a page that I want to save as PDF.
I could printed to PDF or click on the Adobe Icon on the
Explorer, you will get the dialog "Convert Webpage to PDF"
waiting for you toset name & folder.
Ok, everything goes find, but ALWAYS, the name of the page
will be on the very top ( in small case ) and so is the
web Address at the bottom of the page.

BTW: Can't find this on Preferences..

- create a web page from Acrobat - File>Create PDF>From web page
- type any valid URL
- Click settings button, uncheck the option to add headers and footers
- Press OK, and convert the web page
Your preference would be saved and next time you create PDF from IE using Acrobat button or from Acrobat using Create PDF>From web page, you won't see any headers/footers.
