For the root url, you need to specify the url of an existing form. The easiest way to do this is to copy and paste the url from a Form Variable Definition.
The only output variable you need (despite the *'s) is the output document, which should be a document variable.
The documentation is quite accurate and helpful - but you need to read it carefully.

I'm having a little problem with this QPac also. I keep getting a null pointer exception in my stalled error page. Do I have to set all deployment settings for the QPac(JNDI_ProviderURL, JNDI_FACTORY_INITIAL...)?
I set the JNDI_ProviderURL setting to "localhost:1099" and that solved it for me.
I am also trying to use this qpaq with the setValue qpaq in order to create a pdf from xdp and fill a couple form fields with the setValue part..
Im lost at this point and would love a simple example - can someone set me straight here ?
You could try using our RenderPlus QPAC, which is easier to configure than the regular Render QPAC.
