Header and Footer alignment

How do I get the header and footer to align to the left and right no matter whether the window is maximized or minimized.

Example Header: 

(left side) BI Memos                                      (right side) Nisource - Customer Account Processing

I use a table in my footer - just one row, two columns. In the first column, text is left justified. In the second column, text is right justified.

Table is the way to go as stated. What I'm not seeing here is an advisement of creating the table to span 100% of the page width.

Should be obvious, but better safe than sorry.
