Now... a new issue (don't you hate that?)
It seems I've gotten the 'sync virus' that is going around. I've searched quite a bit, and can't determine if anyone has found a solution - maybe there's a 'Sync FAQ' somewhere?
I'm burning Blu-Ray, using the method described (elemental streams, no multiplexing in AME). Burn looks beautiful, but sync gets steadily worse over time. After 10 minutes, nearly 1-second lag. If I export the same project multiplexed, can play in Windows Media Player, syncs great, no probs.
Has anyone figured this one out yet?
(BTW - Encore CS4 on Vista64)

Before I read your post (about audio settings) I re-encoded to h264 (instead of mpeg2), with no multiplexing (creating separate m4v and wav files).
The sync was perfect on the bluray burn, but had some strange random video artifacting which wasn't there in the mpeg2 file.
??

Are you sure it was random? MainConcept's H.264 encoder has been known to generate blocky, ugly keyframes, and sometimes a few ugly frames around the keyframe.
Apparently the problem is that the encoder doesn't allocate enough bits at the keyframe, even if the target/max bit rates are sufficient to produce pristine keyframes. Sorenson Squeeze 5.1 has added a control that can compensate for that, but no such control exists in the AME. I think the best you can do in the AME is increase the distance between the target bit rate and the max bit rate when using the High profile - but that's just an early theory that I have. I only have preliminary data.
As a quick check, try setting the keyframe distance using the Keyframe Every <blank> setting and set it for about 5 seconds worth of frames. Encode a short portion of your video around where you've noticed the artifacting and see if this is indeed the problem.
BTW, you still haven't told me what audio export settings you used that caused the audio length/sync issues initially.

Sorry for the oversight.
My original audio settings (and current) are 48khz PCM. That's where i got the sync problem (with mpeg2), yet sync is fine in h264, but of course the artifacting problem.
A couple of related Q's - would choosing CBR help at all? Also, what about setting keyframes closer together (shooting in the dark here)?
Has anyone had success exporting the video uncompressed and creating in encore that way?

Maybe a simple question: I've re-exported a small segment of the project (per your suggestion above) and when I import the m4v into premiere - no probs - looks great.
Just for kicks i also imported the 'other' m4v that caused the artifacts on the blu-ray burn - also looks fine - no artifacts at all in premiere.
Is this something that i will only see once i burn it? Do i have to coasterize discs to check these files? Is there another way to check them?

You should definitely be using BD-RE discs for test burns, they can be reused over 1000 times.
I actually just build an image, mount it using Daemon Tools, then use Power DVD to playback the mounted image. MUCH faster than burning a whole Blu-ray disc.

When i export using your suggested settings (key frames 5 seconds) and set the target bitrate to 25, max to 35 (vbr 1 pass) when i try to burn or build an iso i get: Blu Ray error "Code 6" 'File Already Exists'; Video Buffer Underflow
This is the same project source as i've been using all along...

I've been trying all sorts of combos - this relates to your mainconcept h264 theory.
I re-burned the exact same file, but changed the Encore project settings to h264 (were defaulted to Mpeg 1st time around) and got the same problem, but the artifacting was at different places in the timeline. I don't know if that info helps you at all.
Also, these clips are all set to 'don't transcode' in Encore, if that matters. If I set the transcode settings to anything (even maximum) the build panel shows my project as being only 500mb, instead of 11gb like it should.... ?
Right now I'm re-exporting to mpeg2 elemental streams with dolby digital audio (back to the beginning). Maybe that will solve the sync problem, and i won't have to deal with the artifact issue....
I'll let you know what happens.

When I try to create an image from a large project, it will predict about 172 hours for the build...?? When i do it for small projects, no prob at all, but the big ones are ridiculous. Have you encountered that?

Sorry to inundate you - just want to keep up to date...
I tried re-exporting the Mpeg (as mentioned above) and still have sync probs. Tried exporting the .wav file independently, still no sync. For the record, the raw wav file is identical to the stream created when exporing elemental streams.
I checked the sync in Encore - and turns out you can see it in the monitor there. At the beginning of the project (45 minutes) it's perfect. By the middle, the audio is ahead of the video (audio event happens before the video). By the end of the project, it's flip-flopped and the audio is BEHIND the video.
So - i'm now exporting as CBR, just a shot in the dark to see if thats it. Obviously the sync problem lies in the video track, not the audio.
At this point, I dont' care if I use mpg or h264, I just want to make my bluray.... oi...

You've covered a lot of ground since my last post!
- You should be able to see the issue in Pr - I can.
- Try playing the H.264 file in either Quicktime player (if you have it installed) or KMPlayer (my favorite "free" player). If you don't see artifacts there, then the MC keyframe issue is not the problem.
- You should try CBR and any other parameter variations you can think of - that's what I'm doing here. In my tests, moving the keyframes closer together just results in more frequent artifacts. But that can be a good thing when you're testing. I'm still researching this issue.
- For your tests, make sure you are using unique file names each and every time you export from Pr; otherwise En will get very fussy and start giving you errors.
- Also, while testing different file lengths and encoding schemes, you should be using a dedicated test project in En. Don't go anywhere near your real project.
- Please heed Jon's advice and use a BD-RE for all of your test burns to disc. As he does, I test disc images by mounting them with SlySoft's VirtualCloneDrive (also free).
- What is your source material in Pr? Is it some form of 24p or 24pa HDV? Is it any form of 24p? If so, make sure it's interpreted correctly in Pr, and then make sure that En interprets the MPEG2 output from Pr correctly.

The issue I was having with the h264 codec appears to have been happening in Enore - not AME.
When exporting m4v's from AME, looked beautiful, previewed beautiful in Encore. The files were set to 'don't transcode' and when burned, funky artifacts at/around the keyframes.
Yet when I used mpeg2, even though it looked great, the sync was off. I even tried exporting lagarith (a chore on a 45 minute project) and MUXing in Encore (12.5 hours), still artifacts.
However, If I imported my AME m4v's into a MPG project in premiere, and forced a re-transcode (which I know isn't optimal, but it looks good so far), then the sync was on and the artifacts were gone.
Apparently, even when the h264's were set to 'do not transcode' Encore was still doing something to them to cause the motion artifacting in the h264 projects.
I'd like to find a proper solution, but I'm just glad this is done, and copy #2 is burning now...
