There are settings that control the badges that appear (or not) on thumbs. They are under the View metadata.
Also, a good place to look at is History in the Develop module.

Ian Lyons Jul 13, 2009 11:11 PM (in response to jrsforums)Lightroom checks for external changes, but can be very slow at doing so. The quickest and by far most foolproof way of checking/updating externally edited files is to use the Synchronize Folder... command (i.e. select relevant folder in Folder panel then choose Synchronize Folder... from Library menu). Using this command forces Lightroom to scan the folder contents for changes. Lightroom will only update the catalog for the files that have been changed externally.
BTW: synchronize folder never saves data back to the files
