Restrict acces to forms made

I have made a form which I want to have displayed on a web page, but I only want approved people to edit and save the form and leave it on display, for general viewing.

Ive been trying and have been partially successful in acheiving what I want but cannot get the form to be displayed for general viewing, what am I doing wrong?
