Comment toolbars in Acrobat Reader apparently changing in a Random fashion

I am coming across a problem with Reader Extension Enabled documents that are produced from Adobe Livecycle. On occasions the comment and markup tool bar changes with certain tools (notably the sticky note) appearing and others (Attach comment as file) disappearing. this happens between sessions of Reader being opened. It also appears to be random in it's timing and occurrence

We are using Reader 8.0

Has anyone come across this sort of problem?

Can you post the document that causes the behaviour?

Unfortunately the documents I have been sent by the users don't reproduce the problem so I don't have a document that would be helpful. I currently have a call out to the users to supply more information and documents
