Brush Preview On Canvas - CS4

I'm having an issue (maybe it's a known bug or just something on my end) when using brushes. After selecting a brush (ie. round with hard edge - or any brush for that matter), depending on my zoom level and brush size, the outline of the brush preview begins to disappear, and even more so when I zoom in further. If I use a 60px round brush with a hard edge, once I zoom to 100%, the round outline of that brush begins to get cropped. When using a 30px round brush with a hard edge, once I zoom to 200%, I get the same effect (see attached image).

I don't know what factors may cause this, but here are some system specs:

Dell XPS Gen 4 with HT

      3.8 GHz processor - 2 Core

      Win XP Media Center v.2005

      Service Pack 3

      video:

      nVidia GeForce 9500 GT

driver version 6.14.11.9107 (which I think is the latest)

I recently had to re-install everything, so maybe I need to update PS. I will try that in the meantime. If it's not one thing with CS4, it's another for me (not to mention the sluggish-ness of Flash CS4 - even after its minor update).

Preferences/Cursors settings don't change anything (normal vs. full brush tip).

Preferences/Performance/GPU Settings is disabled but previous installation of PS CS4 and video drivers allowed OpenGL Drawing - so I don't know why that is sometimes enabled and other times not. On previous install, I would get a message randomly when opening PS that my graphics card didn't allow OpenGL, but at times this was enabled.

So, I don't know where else to look for the answer.
