Just in case it matters: what version (CS3; CS4?); what platform/os (XP, W7, Mac etc).

Verbatim 4.7GB 16x certified DVD+
I build it as image first and the burn to disk with 10-12x speed.

I would burn to a folder and play from the folder version on the computer. If that is okay, it points to disk issues. (Or play your iso on the computer.)
Bill will give you some more detail on the disk issues; try doing a burn at the slowest speed your disks allow. Generally, I like DVD+RW for testing, but if you don't have any....

Only thing that I wonder about is the +R. Nowadays, the type, -R vs +R, as not the issue that they once were. Still, if one has an older set-top player, that could cause some playability issues.
If Play First is working in the ISO, the preview, and from a VIDEO_TS folder on the HDD, but not in a set-top player, I'd definitely check the disc out in another set-top player.
