pkahrel Jun 9, 2010 9:24 AM (in response to mattaca)Ah -- you can do language settings in several places. You probably did this already, but just to make sure: select all text and check what it says in the Character panel. If that doesn't have an English variety, set it there (or change it in the paragraph style). If that doesn't work, I give up.

mattaca Jun 9, 2010 10:42 AM (in response to Jeffrey_Smith)Wow. I didn't even know that existed! Yes, that seems to be the problem. That had been chagned in the template we set up with for this book and got propagated into the chapters we created. It had the second option selected ( ,‘ ) instead of the first (‘’), so there was no option for it to set the apostrophe correctly.
This has occurred at the most random times over the past couple years, and I bet it's because a document gets picked up, scrubbed, and used for another purpose. I would never change that manually. At least now I now how to fix it when it comes up.
