Only the solution I mentioned in my original post i.e. opening up the Tablet Properties application while using Flash CS4 - it seems to wake up the pen tool and it works okay.
Give that a shot as it's the only solution for now.

I'm experiencing the same problem except I'm using CS3. I'm not using a tablet, only a USB mouse. I'll try messing with the mouses properties kinda like you did to see if I can get it working. Thanks for the info. I also get the same bug in Illustrator.
Yeh, I also found out after posting that my mouse was effected too. Unfortunately I've not found a working solution for the mouse
I can see this becoming more of an issue as people upgrade to Windows 7 and therefore I would presume Adobe will have to fix it - now would be good though please!

Same problem here, the system:
Windows 7 pro 64
The problem seems random, sometimes the line being drawn with the pen tool appears in real time, but eventually it ceases to appear, behaving in the fashion related above from them on.
Sometimes alt-tabing to any other window makes it return to normal behavior, but it doesn´t last for long.
The problem affects the mouse too and the solution proposed (letting the pen properties open) doesn´t work for me.
I just tested Illustrator CS4 with the pencil tool and had the same issue. However I managed to make it work by doing the following:
1 - Open Illustrator CS4 with blank page or artwork
2 - Select the pencil tool
3 - Open Wacom Tablet Properties
4 - Using your Wacom pens primary button - click and hold so that the Current Presure bar registers - you will see it grow blue, keep the Wacom Tablet Properties open
5 - As illustrator is open in the background click to focus Illustrator and start drawing
I find this wakes it up and the pen draws in realtime, if you focus another application this can cause probelms so just repeat from step 4.

I found a temp solution to the problem (tested it with Flash CS4)
All you need to do is disable the Windows Aero theme. Instructions can be found here.
This fixed my problem of the pen tool not previewing real-time while drawing. This has not fixed my other problem with the bush tool.
If you have the brush tool selected with pressure sensitivity on, and you click off the flash platform (desktop, any other windows etc...) you will loose the pressure setting anywhere from 4-10 seconds.

Indeed 'Skip1986' is correct as I just received a reply from Adobe regarding my bug report:
We have filed a bug with Microsoft for this issue. It seems to be an issue with their new drawing API. If you switch to Windows Classic desktop theme which downgrades the drawing and graphics capabilites the pen tool draws live again.
So that's the solution for now, until Microsoft can take a look.

uninstall flash air
uninstall adobe flash player10 activex
install adobe flash player10 activex
All of you can try this, turn on Rulers. It can fix the Pencil Tool bug in windows 7. The workaround is tested on Illustrator CS3 and Flash CS3. Good luck.
