Saving Problems

Hi,

How do i upload my document to show you my problem??

My problem is, i created a check box so that if you tick it a new page is added onto it. It looks fine but the problem is when you fill it out and save it it is all mangled! why is this??

Sorry for the simple wording i am not a computer wizz so please help in the most simplest of tems for me to try and solve this!!
