need to be installed in the cs5 12.1 plugins\panels folder.
You should probably run the extension manager as an administrator
(right click on the extension manager shortcut and choose Run as
administrator) and then click on one of the extensions and you will
see something like install to the right of the extension.

the cs5.5 extension manager makes a big show of telling me its converting panels etc and lists them but they don't show up under extensions for the listed programs????

This is very basic, and I apologize in advance if you're already aware, but be sure your 32 bit plug-ins are installed in 32 bit Photoshop, and 64 bit plug-ins are installed in 64 bit Photoshop. They won't show up if mixed up.
