Here is a link to i-nigma with their specs. Although companies may have specifications for reproducing a code, I have found as long as you can capture the entire code on your smart phone screen it will work properly.
As for background: For most readers/smart phones, the code can be reversed to white and placed on a dark background. But I have had some readers not work.
As for verbage, I have used taglines similiar to this: "Scan QR code with a free code reader on your smart phone to go directly to..."

I've actually never scanned a QR code (I guess I need to get with the times!). Does that sound reasonable for scanning with a phone?
Thanks for the verbage. I guess I should look for one of those readers myself and install it so I know what's going on with this.

If you go to www.i-nigma.mobi with your phone, the site will detect the type of phone you are using and download the appropriate reader

And if the code doesn't take you to a mobile-device-friendly destination, then it is a real waste of time. QR codes can be useful, like a contact card that can be exported and saved to address book.

It either works or it doesn't. Should be fine at 1". Be sure to download a QR reader for your phone so you can scan the code for testing.

Test the QR code at various sizes using 2 or 3 different scanners and models of phone.
They can be test on the monitor. In InDesign press CTRL/CMD 1 to view "actual size" it's approximate but it will do. And scan it. And also print it out.
