George_Johnson Aug 19, 2011 12:06 AM (in response to RoyceStringer)Those PDF forms are almost certainly created with LiveCycle Designer, as opposed to Acrobat. Unfortunately, forms created in Acrobat do not allow you to create a graphic/image field that will work with Reader. LiveCycle Designer allows you to add image fields to a form, but it is not included with the Mac version of Acrobat like it is with the Windows version.
This is where I insert my plea to post a feature request to bring this functionality back to Reader. You can add to this one and/or create your own: http://forums.adobe.com/thread/743823?tstart=0

George_Johnson Aug 20, 2011 9:47 AM (in response to RoyceStringer)It's been available with Acrobat since JavaScript was introduced with the form plugin update to 3.01 (1998). The problem is it (i.e., the field.buttonImportIcon JavaScript method) is no longer available with Reader, from which it was unceremoniously yanked when Reader 6 was released. I'm perennially drumming up support in hope of its return.
