Recommended solution for missing TOC not working

So--why does a help project with the following text at the top of the .hhp not show a TOC or index, only search, in the .chm? I've run through the steps in the tech database note several times, and this is what it's looked like every time, but no TOC or index.

Those are the two I've been working off of, but with little luck.

I decided that instead of trying to understand why it's not working, I should just go back to the last version that did work and import/replace topic files as needed. So I did, and it works. I'm trying not to let the fact that the hhp file and such look exactly the same bother me.

The HHC and HHK files are automatically generated by RH as part of any project.

1. They're blank if you haven't added anything to them.
2. They won't appear unless you tell RH (in the Generate wizard) to include them in your output.

The TOC has content, and the Index is actually topic-based. Wouldn't selecting "default" in the wizard produce them, though? We only have one window defined.
