Steve Werner Oct 14, 2012 6:53 PM (in response to mckinney3)I'd start with the usual troubleshooting methods: (1) Restore your preferences (2) Export the InDesign file as an IDML file to deal with possible corruption.
That should be happening. Hasn't happened to me in CS5.5 or CS6 when exporting EPUB.

Alas I still have the problem. Let me be more precise.
I have a indd file that is very simple and follows an inline structure with all paragraphs being styled, etc.
Page 1 is a placed greyscale jpg file (all one image) that is the size of the document page.
I define TOC and have all my Chapter Headings correctly styled.
I then export as an epub file and first just click on the cover option to be a rastersized first page.
All goes exactly as it should. My Cover is the Title page and the Title Page is intact along with a TOC and page breaks at my chapter headings.
I can substitute a color Title page (the one I really want to be my Book Cover for, say, an iBooks shelf) and all is still OK.
Then I use the grey scale title page.
Export as epub
Then I click on the Cover Page option in the export dialog to use an external file for the cover.
Immediately, the information in the box for Publisher goes away as well as my selections for TOC and Page Breaks.
I refill those boxes and radio buttons and then export.
The exported file has no TOC and all of the page breaks at chapter headings goes away.
All help and suggestions greatly appreciated!

Exact same thing happens.
I created a new user. Logged out and logged back in as new user. Createrd brand new document
Exact thing happens. Trying to use an external file for the cover clobbers the export document as before.

Steve Werner Oct 15, 2012 6:00 PM (in response to mckinney3)What file format is the grayscale image? What color mode? It may be that InDesign CS5.5 is expecting an RGB JPEG or PNG file.
Try opening the grayscale image in Photoshop and convert to RGB. It will still look grayscale.

It's a limitation in InDesign CS5.5. It's not doing what it's supposed to be doing. It's not really writing the XHTML and the CSS so that your independent cover image can be used.
I was using InDesign CS5.5 when I was learning to create EPUB. I largely used writings by Elizabeth Castro and videos by Anne-Marie Concepcion. In Elizabeth's excellent first EPUB about creating EPUB, she describes how you created a cover (in CS5 days) by creating an XHTML file for the cover, and writing the code (she showed you how), and how you had to make a few changes in the other files I'll describe before.
When I first started creating EPUBs CS5.5 had the Rasterize First Page method. I never tried the independent cover file method. Anne-Marie doesn't even show it in her InDesign CS5.5 to EPUB video.
I was only when I started using CS6, that I tried the independent method and it worked. I never tried it in CS5.5. Not everything is written correctly.
So if you want to continue to use CS5.5, you'll need to create is by modifying code. I'd suggest picking up Elizabeth Castro's EPUB. Here's the link:

You'll find directions in the Inside an EPUB file > Creating the cover.

You'll need to use an EPUB editor to add the "CoverImage.xhtml" file to the OEBPS folder.
Then you'll need to add your JPEG file to the "image" folder in the OEBPS (or make an image folder)
Then you'll need to edit three parts of the "content.opf" file: To the metadata, to the manifest, and to the spine.
Or, alternatively, you can upgrade to CS6, which is what I would recommend.

The file you sent that was rendered by CS 6 is still not behaving. The simple title page (grey and white) in the temporary.indd file which is page 1 and what I call the title page is gone. The red external file is now page 1. Rasterizing this title page 1 has always worked properly for producing a cover. The TOC and Page Breaks are OK.
Not sure what you rendered in CS6 but the outcome is still odd.
Page 1 should be the grey and white graphic that is existing page 1 in the file temporary.indd. The exported file should show up with the red external file as the "Bookshelf" cover image. The toc and page beaks should remain intact.
So I am still in a quandry about upgrading to CS6 as this doesn't seem to work either.
What are you seeing on your readers with the file you exported from CS6?

mckinney3 Oct 17, 2012 7:58 PM (in response to mckinney3)I tried adding a Cover using the tools in Calibre. as a work around. Calibre locates the external file and adds it to the book as new page 1. The original grey title page is moved to page 2 and so on.
This does create the "good looking" Book cover for bookshelves and retains toc, etc but the same thing can just be done by adding that page in the original indesign document and specifying to rasterize page 1 as the cover in the export dialog.
Can anyone verify that CS6 will retain an original page 1 , keep toc and breaks based on paragrph style AND add a Book cover from an external file as part of the meta data so the graphic shows up on a book shelf but NOT in the book as displayed by a epub reader?

Steve Werner Oct 18, 2012 5:18 AM (in response to mckinney3)I never had any problem retaining paragraph styles and TOC in any of the file you sent me when I tested in ADE.
If you use the external file as I suggested, you do need to delete the extra page which also used when using the rasterize method.
Unfortunately, I'm travelling today and have classes tomorrow so won't be able to do any more testing for a few days.

I guess we need to close this thread with the conclusion that Indesign CS5.5 has some shortfalls as far as epub exporting. And that CS6 is a step forward.
I find it strange, though that their are many videos and "guru" help blogs that show how to use an external file for a cover in 5.5. I guess they don't really test their work; just want to retread other articles to get words down and attract "ads." =(
