You would be better to place your cache on a seaparate, fast, drive. Create an Adobe folder where you want them to be. Go to Edit, Preferences, Media tab, and there you will see two "Browse" buttons. Use each one to navigate to the Adobe folder you just created and click OK..

Does the button not actually do anything?
For some reason the 'Clean' button may not delete cache files for projects other than you're currently working on. It also never deletes .ims files...

Can I safely delete them manually?
Yes. On a next start PrPro will rebuild cache files for those assets, which are currently in use in your project.
What is more, cleaning both Media Cache and Media Cache Files folders manually helps sometimes resolve some Importer's or Dynamic Link issues.

There are two sets of files here, the Media Cache, which can be quite large and take up considerable room, and the Media Cache Database, which are pretty small files. The Clean button only clears the small Database files. You have to manually delete the larger Cache files.

That's not quite accurate, although quite close. Closer, than my previous statement. The 'Clean' button does sometimes delete some CFA files (which are the largest ones) in Media Cache Files folder. In particular, CFA files related to offline files in a working project. That doesn't change the whole picture though.
Paraphrasing Toomany3, the button should have been called 'Partially Clean Database Randomly'.
P.S. Nope, my previous statement was closer, since I didn't say 'does not', but rather 'may not' (delete cache files for projects other than you're currently working on).
