How to buy FormsCentral Team license

I'm trying to buy the Team license for FormsCentral but I couldn't find any way to do that.

Team documentation page http://forums.adobe.com/docs/DOC-2903 did not help, links are not working for me.

I'm using the Plus license now.

Could someone help please? Thanks in advance.

We no longer provide the team plan. You can just purchase multiple Plus subscriptions (there wasn't a discount for the 5 seat team plan so you don't really lose anything)
