For a web form, it should show three tabs, labeled "Web Form", "PDF Form", and "Embed Form".
But if your form is a PDF form and not a web form, you will only have the options you mentioned. Since PDF forms are not hs=osted on the FormsCentral Server, you are free to make them available however you want. I would suggest not trying to embed a PDF in a web page since it simply won't work for a number of users.

AshBinns Oct 14, 2013 8:02 AM (in response to George Johnson)Yes it is a PDF form. Can I convert a PDF Form into a web form usng formcentral? Or would I use Acrobat XI? My goal is to have a webform that users can easily use on my website.

George Johnson Oct 14, 2013 8:57 AM (in response to AshBinns)You can't automatically convert a PDF form to a web form, but you can use FormsCentral to manually recreate it as a web form. Take a look the the templates that are available with FormsCentral to get an idea of what's possible.

I have tried to find something on Adobe's webpages, but haven't been able to find a process for Acrobat Pro XI that actually works.

FormsCentral will not create a web form of your PDF for you, the Import PDF feature does not provide a web form of the PDF. Acrobat Pro XI also will not convert a PDF to a web form.
What George was saying is that you can use the FormsCentral form creation tools and either start from a Template similar to the form you want to create, or start from scratch, and re-create the form you have as a PDF in our system, adding the fields to the form to as closely match your PDF. I took the time to create the Web Form, since the PDF Form wasn't going to allow me to redirect to a different URL after clicking on submit, so I finally did get a great Web Form created. NOW, I cannot move the Submit button to the right side of the Web Form. There isn't even an option where to put the Submit button. I would like the Web Form to match the design of my webpage, but if I cannot even pick where to put the submit button...
FormsCentral sucks. Anyone know a better Form hosting solution?? I'm all ears.
