I'm on CC and windows but I'm sure it's the same on a Mac.
Go to Preferences/Audio Hardware/Output Mapping and open the HDMI output in the 'Mao Output For' box You can then select which Premiere channel goes to which audio output.
Yeah I tried that as well. I even tried to rearrange the channel orders inside premiere but it didn't change at all. Doesn't seem like changes I do is reflected, I don't think it's downmixing to stereo either because I set it all to "Front, back, and LFE".

I know it's nearly a year later, but I am having almost the exact same issue that you were having reconfig2501. It seems to still be present on Premiere Pro CC (2014) on Windows 8.1. It's great that Premiere is allowing 5.1 audio tracks, but how am I supposed to preview the 5.1 audio on my timeline? If I am editing for HD Broadcast that supports 5.1 channel audio, I shouldn't have to export to ac3 and burn a Blu-Ray/DVD just to preview the 5.1 audio.
