my forms and data are gone...I need immediate help restoring

Hi;

Pretty much every time this happens the user has signed in using a different email than the one that they had signed in with previously. 

Try signing in with any other email you could have used to sign up for an AdobeID with, this is a very common occurance and is always resolved by signing in with the right ID.

In your case I would try signing in with another email address.
