Where can I get assistance with filling out the Runtime Distribution Form?

I'm trying to renew my runtime distribution license and can't figure out what they are looking for in the "Please provide the product or service name and description" box for each product. 

Any assistance would be appreciated.

Adobe's distribution license applies to:

- Flash Player (Plugin)

- Shockwave Player (Plugin)

- Adobe Reader (Installer)

- Adobe AIR (Runtime Installer)

In your case the "product or service name and description" would be "Flash Player  (Plugin)". If you distribute anything else, separate it with a comma, and add the name from the list above.
