Preview, however, should not be used for PDF forms (and certain other PDFs) since it irreversibly damages them in a number of ways. Use Adobe Reader instead.
A typical symptom of a form that's been corrupted by Preview is the text fields appear to be blank when opened in Reader, but the field contents can be seen when a field receives the focus.

The Real Thorzdad Dec 27, 2013 3:45 PM (in response to George_Johnson)I tested it in Preview because it's a good chance that many of my client's customers would also be using Preivew. As I said, I also tested it in Reader, and it also worked on my end. But, my client had the same problem with the checkboxes when using Reader.

try67 Dec 28, 2013 2:04 AM (in response to The Real Thorzdad)Just by opening the file in Preview it can get corrupt. I've seen it convert check-boxes into radio-buttons, change the properties of fields and of course cause the values in them to become hidden.
You should not use Preview to open or fill in PDF files under any circumstances. Doing so is asking for problems. Instruct your users to use Adobe Reader.
