One of our lead Prelude QE will reach out to you. They may want to peek at your folder structure and system settings to help better understand the environment you are in.
The only known issue with crashing on ingest is related to the missing folders which the installer was failing to create. This is fixed in our upcoming release. You do mention that you are aware of this issue and it didn't fix your problem. We will work with you to identify the core issue and resolve it.

Could you please try below steps for me and let me know the result?
1. Please go to Your Disk:\Users\Public\Public Documents\, clear(or rename to 'Adobe1" if you want to backup the existed folders) the folders under "Public Documents"
2. Re-launch Prelude to let Prelude re-creates the folders
3. Try the "ingest" again

How would I go about providing that?

I faced the same issue, I opendI did all steps you suggest with the only difference I renamed ADOBE in mi local directory C:\Users\vimere\Documents\Adobe even if I also see the same folder in Public folder C:\Users\Public\Documents but once I delete it it willl never appear again. Maybe is a refuse of my old Adobe installation (CS6, CC8).
My licence is a company one for VIacom

Unfortunately I cannot format my laptop but I copied the same installer ver 2.8.1.451.The tool installed the old release 2014 with the same issue.
I've been heard that the only way to instal the lates version is format the HDD?
Is it true?

SOUTH_Director Nov 9, 2014 5:32 AM (in response to SOUTH_Director)As we use to say in Italy "me la suono e me la canto" translated "I play myself and I sing myself" which means I raise the question, I eas hungry for that but then I was the cause of the problem.
Here how it has been solved.
Under menu Edit / Preferences / General / media
click on clean "Media Cache Database". It take a while to reply particularly if your foder is quiete full like was mine (30GB)!!
Once cleaned everythinhg goes fine.
I did probably something wrong, I've been using Prelude a lot but I didn't know I should have cleaned. I'll ask to my to my Adobe Tutor Paolo Baccolo.

SOUTH_Director Nov 10, 2014 3:32 AM (in response to emanuele de capitani)once I solved the issue deleting the cache database I've been worked for few hours and then Prelude crashed again. I Update CC to lates version and now ... Premiere CC 2014.1 crashes too
Someone suggested me to rebuild the workstation. Is this the only way to fix the issue?
I tried also to update graphic card but nothing has been changed
