Use my own pdf form as a template

Hi;

FormsCentral does not enable you to edit a PDF - there is an "Import PDF" feature that allows you to use an existing PDF form and import it into FormsCentral to collect and manage responses, but it does not allow you to edit and does not provide a "web" version of the form.

You could use Adobe Acrobat XI to edit your PDF adding a drop down list.
