Installing/Activating Flash Player Plug-in

Since I upgraded to Flash Player 13 I have not been able to view videos, etc.  I have IE 9 running on Windows Vista SP2.  I looked at IEs Manage Add-ons and see that Flash Player is Enabled but there is no "install time" listed.  I double checked the Flash Player Setting Manager in Windows Control Panel and it tells me that the ActiveX version 13.0.0.182 is installed but that the plug-in version is not. 

How do I install/enable/activate the plug-in??
