java script radio button then a textbox is shown/ Help me modify this code

Hello
found this snippets on the web. hope you can help me modify it.
so it can
show a unique textbox1 based on a radio button1 selected.
for example
if radio button is selected then a corresponding textbox is also selected. like wise if radiobutton2 is selected then textbox2 is shown
and so on


currently it just shows a value inside a textbox. Help me modify this code.
