Does anyone know if there is a way for a person who receives (and is reading a .chm file) ---a way for the reader to type comments at certain points and then send the file back to the originator?

Not quite what you want but output a printed version at the same time, (which will be a Word document).

In the Word document, go to the tools menu and select "track changes".

When you get the document back you should be able to see any changes made by reviewers.
