How do I make a navigable PDF file?

I have a few different PDF files that I want to make into one, navigable document. It's sort of like an instruction manual. It will have a table of contents, and if people want to jump to chapter 2, I would like for them to click the link in the table of contents and have it jump to that particular section. Is there an easy way to do this? I can't find anything online to help me. Hopefully someone here can. Thanks!

Draw a link around the words "Chapter 2" with the Link tool.
In the Create Link dialog that opens, you can choose "Go to a page view" to navigate to a page in the current PDF or another PDF that is open.
Click Next
Navigate to the destination page
Click Set Link.
Test the link by clicking it with the Hand tool.
