How do I create a customizable form?

I'm currently working on making a website in Muse for my company J-TEK PC's and I'm trying to make a form like that on HP or DELL's websites that allows you to select different options then bring you to a unique check out page. How do I make a customizable form?

Hi,

If your customization includes features that are not currently offered by the Form widget in Muse then I would suggest you to create your custom form in any other application like "Adobe Form Central " Or " Jotforms " and then use the embed code of the form created in your muse site.
