So I will open a PSD with an adjustment layer.
I turn off and on the "eyeball" in the layers panel. It counts as a change.
If you look at your document tab, you see a star at the end of the filename. It shows you made changes to the file.
File > Save would remove that star and File > Save does dim on my Macbook.
If you are seeing it twice, the Preferences file may be corrupt. I suggest a reset.
Macintosh - CMD-SHIFT-OPTION immediately after you double-click the PS icon.
If that doesn't work uninstall/reinstall CC 2014 and updates'

onyonet Nov 24, 2014 9:01 AM (in response to gener7)The reset didn't work. I'm not willing to reinstall PS unless I have perfect instructions how not to lose all my settings and actions. I can live with the way it is more than I can live without those things.

gener7 Nov 24, 2014 9:34 AM (in response to onyonet)If you mean Custom Presets and Actions, it should not touch them, but you might gather those into a backup folder.
One trick I use is to keep a Presets folder with all my custom stuff in the Documents folder. It helps when I change Photoshop versions.
I can simply point to the Documents/Preset folder instead of moving them to the new install's Presets folder.
So don't consider an uninstall until you have backed up to your satisfaction. Custom keyboard and dialog settings also have to be considered.
If repeating the Save command is a minor irritation, it's best to leave it alone for now. A patch might come along and take care of it.
I also strongly advise setting up Time Machine. A 256 layer file you are working on gets corrupted, you go into TM and pull out an hour old copy instead of starting over.
