Do you see field highlighting in light blue color in your PDF form?
Could you check if the PDF form is opened in Adobe Reader for iOS? When you tap anywhere in the PDF form to show the toolbar, what is the color of the icons/buttons? Blue or "salmon pink"?
Apple Mail, Safari, and iBooks on iPhone/iPad can display the preview of a PDF document. However, because Apple preview does not support forms or annotations (sticky notes, highlight, underline, etc.), filled data and annotations appear blank or empty.
Once you find out which app is displaying the PDF form, you can use the "Open In" feature to open it in Adobe Reader for iOS.
