Chronic installation issue with iMac OS X 10.9.5 with Safari

I have an iMac with OS S 10.9.5 with Safari, I tried the upgrade and got to 90% and then the install failed.  I have tried the uninstall and then a clean install and still just get to 90% and then it fails.

I have verified multiple times that the flash player does not exist on my computer anywhere.

I have shut down and restarted in between steps.

Please help!!!  Very frustrated!

I missed three class last week because the tool they were using used flash player now I am a week behind and still no solution.

I apologize for not replying sooner.

This is the error resulting in the failure:

installer: Error trying to locate volume at /

Unfortunately, it looks like something is messed up with the filesystem and the installer can't see the root drive to install to.  A Google search returned numerous results affecting various products and the recurring theme is that something is messed up with the disk.  There are many recommendations from various sources, but nothing definitive.

I would try the following options:

1. Try using the PKG installer.  This is geared towards Mac SysAdmins to install Flash Player within their organization.  It uses a slightly different installer path, which may or may not work.
2. For the offline installer (install_flash_player_osx.dmg), some people have had success with disk and permissions repair some have not.  My recommendation would be run a disk and permissions repair.  Reboot the system after performing the repairs and try to install again.

If these options do not work, I'd reach out to Apple support to see if they have a suggestion.  If you do find a resolution to this, please let me know.  I'll be happy to share it with people that encounter the problem in the future.
