Webapp with 2 mandatory search fields.

Good day,

How would I create a webapp search bar with 2 search parameters?

For example, if one search field is 'Customer name' and another search field is 'Customer ID' I need both to be mandatory for it return the the result. This is to stop the webapp search module from returning 3 names if there are 3 customers with the same name eg, 'John Smith' the 'customer ID' will also need to be present in the search bar.
