Maybe someone can help me out with this.

I wanted a consistent filename for this survey form that is frequently used by my company.
I could never get the file to name itself so the next best thing is to provide a helper field at the bottom which enables a technician to cut and paste the filename as he signs the form.
The filename should appear but not print when all the variables involved have values.
At this time it is not working.
Can someone assist me please?
I wish the filename and the helper field to be a specific format.
