i add new pages to my multi-page document in the centre section. i then have to move them into position as it doesn't insert them as new spreads ie. if i add 2 pages after page 123, it adds them to the side of page 123 and not underneath as a new 2 page spread.
I then get error message "Internal error communicating with AI rulebase . Layout adjustment could not be performed."
Indesign goes into 'wireframe' mode. If i press W it alts between preview and wireframe mode but eventually performs as normal - ie i can press W to change between preview and normal mode.

Have you tried turning spread shuffling back on? If the pages are not making new spreads it seems highly likely that you have disables shuffling for at least the spread containing page 123.
'Allow selected spread to shuffle' is checked.
'Allow document pages to shuffle' is unchecked. - may be try checking this?
I had this same issue or error message while trying to change the page size within Document Setup from A5 to another format. But I just randomly found a solution (for my case). The original version had "Facing pages" option checked. What I did was uncheck that option and click ok. Then went back to Document Setup, changed the Page Size, and clicked ok. It worked perfectly. After that, I could even check the "Facing pages" option again. Apparently, resizing the document while having that option checked, creates an error.
