Error adding payment method

I'm unable to add a payment method to my account and keep getting the error message below.  I get the error on Windows 7 Firefox, Chrome, and IE.  Also get the error on OSX 10.11 Safari.  My plan is the "Creative Cloud Student and Teacher edition (one-year, prepaid)"‎.  Anybody else having this issue?
