Problems with Accessibility Checker

I am making PDFs 508 compliant (complying with ADA requirements). Using the accessibility checker, I perform a full accessibility check. After I perform the accessibility check, random characters appear in the PDF -- usually exclamation points. I can remove the exclamation points, however, the formatting is affected. Has anyone seen this and is there a solution?

Could you please confirm the version of Acrobat installed on the machine?

Try to run accessibility check on another pdf and check if the file produces the same result.
