It says that an extended license for an image from Adobe Stock cost $79.99 + tax and that you get one per month? Is the $79.99 paid only once, per month, or per image?

You can get as many Extended licenses as you want. There is no one a month limit. I believe what you are reading is that the extended licenses are only available as single purchases and not available as credits from your standard subscription. The $79.99 is a one time cost per image. You are not limited to one a month.
