whenever I click on the flat watercolor brush, theres always one fan shaped icon pops up at the end of toolbox.
Can anyone tell me what is that for?

It's a fan to "dry" the watercolor. If you watch the ink, it will keep spreading until you either draw on top of it with ink, close out of the drawing, or tap the fan icon.

Is there a brush that acts like this in photoshop? Something that spreads?

Hi Chelsey.
I know that there are a couple of artists (Kyle Webster is one of them) who make watercolor brushes for Photoshop (https://www.kylebrush.com/assets/cheatsheets/KyleRealWCcheat0915.pdf) but I don't believe that even those "spread" like the watercolor brush in Sketch.
