Adding a permanent label to PDF

Question- I am using Adobe Acrobat Pro DC and I either scan a document or download a PDF document and add a permanent label to it.

Sometimes it works and other times it will reformat the whole document.  Is there a way to add a label (or text box) to the document without it reformatting the original document?
