LR4 Not Creating folder

Hi I'm have a problem with import , with i try to import photos and i select destination and check into subfolders and put a title in and organize by date and the date format has todays date . then i say import it creates and other subtitle and uses and date of 2014 ?why is this

Give us the exact details. What are the settings in the Import dialog box for the Destination (show us a screen capture). What is the exact result ... the exact name of the folder created ... and any other detail that is relevant ... leave nothing out.

thanks here are the screen shots. what you can see is it keeps creating 2014 folders?
