crashing Photoshop CC2015 when create a new dialog window

hi all.

I have created a toolbar in HTML, when clicking on the icon, you need to call a new dialog box with the settings. On one less-powerful computer when you call window is shut Photoshop.

Help to understand what was going on and how to fix the problem ?

if you paste this script in EST, then the window is created normally, and if you call from the panel, then Photoshop is crashing
