Not sure what exactly is causing this, since it happens when I take a step back and / or use certain tools.
This is what is happening:
Before re-installing (after using the Cleaner Tool) you should at least try restoring Preferences after making sure all customized presets like Actions, Patterns, Brushes etc. have been saved. (explained in the above link)

Does turning off »Use Graphics Processor« in the Performance Preferences (Photoshop > Preferences > Performance > Graphic Processor Settings) and restarting Photoshop have any bearing on the issue?

Just to make sure: You did restart Photoshop after changing the setting?
What are your GPU Preferences?
What have you done for trouble-shooting so far?

I did restart, yes.
As far as trouble-shooting, I have been changing performance preferences but I don't have control over plugins or updates (I work for a University).
My hope was that this was a preference issue, but I may just need a re-installation of Photoshop.
