Is there any way when exporting a video file (.mov) from Lightroom to include the metadata?  I would like to include title, caption and keywords.  I am currently using Lightroom 5.7 for Windows 7.

LR doesn't implement this, unfortunately.  You could do it using the Run Any Command plugin and the free ExifTool utility, but the learning curve on ExifTool is a little steep.  But ExifTool would write the the metadata fields into the XMP section of metadata, not the QuickTime section (assuming you're exporting an MP4), a place where most other apps wouldn't see it.
