I think if you post some screen captures we could see what you see better than  a picture you take.  Here is what I see with CS6 Liquify.

Did you update video drivers recently? If so, you may want to back grade if it was working before.

Post your Photoshop menu Help>System Info you need to provide some information about your environment..... CS6 is very old....
