I have the Adobe Acrobat standard DC plan.

When I go to the organise pages tool I am not given the option to open only to add.

When I click on add I am taken on line to upload and organise the pdf. Can I not do this offline?

Also I have a pdf which is a book with 200 pages and has already been edited. When I open the comments tool to edit it is impossibly slow and often reverts to not responding. With other pdf files it works ok. Any reason for this?

I think there is a good chance you still have the Acrobat Reader app installed, rather than (or as well as) Acrobat Standard. Just buying the plan doesn't replace your software.
